The Office of Licensing and Monitoring requires all licensed private providers to submit a Safety Report monthly. Safety Reports are due on the 10th of every month (unless the 10th falls on a weekend or state holiday and then it would be due by the next state business day).

OLM allows five business days for agencies to make corrections to their Safety Reports if report was sent prior to or on the due date (10th of each month). It is the provider agency’s responsibility to submit accurate and timely reports.

To comply with Department, State and Federal Requirements regarding the protection of Personally Identifiable Information (PII) (i.e. full name, date of birth, address, etc), the Office of Licensing and Monitoring (OLM) requires:
The monthly Safety Report MUST be encrypted and password protected. Unprotected Safety Reports will NOT be accepted as a completed monthly spreadsheet report. Please ensure that the password is entered exactly as provided by OLM or your agency’s Safety Report will not be able to be accessed by our office. In addition, if OLM has additional questions regarding the Safety Report, it will be returned to you via email as an encrypted document utilizing the same password.
