The Maryland Department Human Services is committed to providing access and reasonable accommodation to individuals with disabilities in the delivery of its activities, programs and services.

You may print out the form and take it with you to your appointment. Or, you may email or fax the form to the Customer Access Coordinator at your local Department of Social Services prior to your arrival. Customer Access Coordinators are responsible for assisting customers with reasonable accommodation requests.
